Spay / Neuter Application
Lakeshore Humane Society Spay/Neuter Assistance Program is a reimbursement program to pet owners to aid in the costs of spay and neuter procedures for their dogs and cats. Effective immediately, a tiered system of reimbursement to pet owners at/below the federal poverty level guidelines is $150 for a neuter or a spay. Reimbursements to pet owners above the federal poverty level is $75 for a neuter or a spay. Proof of financial need is required to receive the higher reimbursement. Accepted forms of proof include Social Security benefit letter, pay stubs, current SNAP benefit letter, or other qualifying documents. You must include a copy of your financial documentation when mailing back this form, or attach a photo of same if submitting on-line.
Note: must be a resident of these cities/villages/townships in northern Chautauqua County.

INSTRUCTIONS FOR COMPLETION OF THE LHS SPAY/NEUTER APPLICATION
PLEASE NOTE: LHS Spay and Neuter Assistance is not a rebate program. You must submit this application and have the application reviewed, signed and returned to you by the LHS Spay/Neuter Treasurer before your pet is altered. If you have already had your pet spayed or neutered, you are not eligible for reimbursement under this program. No exceptions to this guideline will be given.
1. Pet owner completes only Section A. on the front of this application. Please return the form, along with supporting financial documents if appropriate, and include a stamped self-addressed envelope so the form can be returned to you once the LHS Spay/Neuter Treasurer has completed Section B. You may complete the application on-line and attach supporting financial documentation, if appropriate.
2. You will receive this application back in approximately 1-2 weeks from the LHS Spay/Neuter Treasurer. The application will note the amount that LHS will fund for your pet’s procedure. The higher reimbursement rate will only be provided to pet owners submitting acceptable documents proving financial need. If you have an urgent situation and feel that you cannot wait for this form to be returned to you in the typical timeframe, please call us at 716-672-1991 and someone will assist you.
3. Take the authorized form with you on the day that your pet is spayed or neutered. Your veterinarian will complete Section C. of this application and return it to Lakeshore Humane Society. The authorized amount will be paid directly to the Veterinarian (Clinic/Veterinary Hospital) who performs the procedure. You, as the pet owner, are responsible for the remaining amount of the veterinary bill.
Due to limited funding, we offer assistance to pet owners residing in the following townships, cities, and villages contained within: Arkwright, Brocton, Charlotte, Cassadaga, Chautauqua, Dunkirk, Forestville, Fredonia, Hamlet, Hanover, Irving, Mayville, Pomfret, Portland, Ripley, Sheridan, Silver Creek, Sinclairville, Stockton, Villenova, and Westfield.
