INSTRUCTIONS FOR COMPLETION OF THE LHS SPAY/NEUTER APPLICATION

PLEASE NOTE: LHS Spay and Neuter Assistance is not a rebate program. You must submit this application and have the application reviewed, signed and returned to you by the LHS Spay/Neuter Treasurer before your pet is altered. If you have already had your pet spayed or neutered, you are not eligible for reimbursement under this program. No exceptions to this guideline will be given.

1. Pet owner completes only Section A. on the front of this application. Please return the form, along with supporting financial documents if appropriate, and include a stamped self-addressed envelope so the form can be returned to you once the LHS Spay/Neuter Treasurer has completed Section B. You may complete the application on-line and attach supporting financial documentation, if appropriate.

2. You will receive this application back in approximately 1-2 weeks from the LHS Spay/Neuter Treasurer. The application will note the amount that LHS will fund for your pet’s procedure. The higher reimbursement rate will only be provided to pet owners submitting acceptable documents proving financial need.
